Make a referral

If you feel your patient may benefit from St Luke's Hospice's services, this page will give you a quick guide to referring them to us.

The information below is for healthcare professionals. If you are a patient or loved one and would like to access our services, please speak to a healthcare professional involved in your care, for example, your GP or district nurse.

How to make a referral

At the links below, you will be able to find our referral form as well as our referral criteria document for guidance on eligibility. 

Please carefully read the referral criteria document to ensure that the patient you are referring is eligible for our services, and help aid you as professionals in choosing when and who in the team to refer the patient to. Please ensure that the patient has consented to the referral and the sharing of their clinical records with St Luke's.

Please complete the referral form and tick the relevant service you wish to refer to (In Patient Centre admission, Community Specialist Palliative Care team or Patient and Family Support Service).

Once the form is completed, this should be emailed to [email protected]. or posted to us at:
St Luke's Hospice, Little Common Lane, Sheffield, S11 9NE and marked FAO. Clinical Administration team.

Please note, any incomplete forms may result in a delay.


Next steps

  • Once we have received your referral, we may contact you, as the referrer, to get more information if required. Please ensure your contact details are up to date to avoid any delays.

  • If your referral meets our criteria, we will then contact the patient or their next of kin to carry out an assessment. This will help to identify the patient’s needs and if they have any concerns that we can help with. We may also be able to signpost or refer to other services if needed.
