Becoming a patient

In order to receive our care, you need to be referred to us by a health professional. Here's how it works.

In order to be referred to St Luke's, you need to have been diagnosed with a serious and progressive illness for which there is no known cure. The illness must also be well-advanced and not in its early stages.

We can’t cure you, but we can help you to live as normally as possible for as long as possible, by providing specialist medical and nursing care, advice and support when, and how, you need it. Many patients live for a significant time following their referral to us.

Once referred, there are various ways in which you can access our services. Initially, we would offer you the one which best suits your needs at the time, but our flexible approach enables you to switch between them, should your needs change.

Our Patient and Family Support services provides specialist palliative day care to patients with complex needs through support and access to tailored programmes of activities, therapies, and treatments, as well as nursing and medicinal advice over an 8-12 week period.

Our Specialist Palliative Care Community Team supports and advises patients living at home. They work closely with GPs and district nurses.

Our Specialist Palliative Care In Patient Centre is for people who are extremely ill or whose symptoms require 24-hour monitoring and intensive specialist palliative care.
