Transition Service at St Luke's
A growing number of children and young adults with life-limiting and life-threatening conditions are now living into adulthood, meaning they are now transitioning from children’s services to adult services.
We are aware this can be a very stressful time for young people and their families; at St Luke’s we are committed to supporting the transition process for all young people moving from children’s palliative care to adult palliative care that require our support (as well as those individuals diagnosed with a life limiting condition after 16 years old who may not be known to children’s services).
We work closely with Bluebell Wood Children’s Hospice, Sheffield Children’s Hospital and Sheffield Teaching Hospitals in trying to ensure each young person receives the support they require as they make this transition.
At present we are regulated to care for patients over the age of 18 years of age, but we are aware that after the age of 16 years, young people are considered as adults within the healthcare setting and this is something we are working to address. If you wish to refer or discuss a referral for a young person between the age of 16 and 18 years, please contact Helen Smith, Nurse Consultant in Palliative Care, on 0114 235 7663 or email helen.smith181@nhs.net.
We can offer the following
Symptom management
Counselling
Art Therapy
Emotional support
Support for the family
Physiotherapy
End of life care
Family suite
Bereavement support
Our symptom management and end of life care are offered both at home or on our In Patient Centre at the hospice.
Please note: St Luke's does not provide respite care.
